Baclofen and alcohol in France (The Lancet, 2018)

Ende 2018 veröffentlichte die „Elite“ der Baclofenforschung mit Schwerpunkt Alkoholabhängigkeit in der renommierten Fachzeitschrift „The Lancet“ das so genannte „Cagliari-Statement“ als Reaktion auf eine systematische Meta-Analyse von Tom Bschor und Kollegen. In diesem Statement schildern die Autoren den aktuellen Stand der Behandlung von Alkoholabhängigkeit mit Baclofen und sprechen sich für den Off-Label-Einsatz des Medikaments aus – nicht zuletzt aufgrund mangelnder effektiver Alternativen.
Florian Naudet & Alain Braillon wiederum ließen das „Cagliari-Statement“ nicht unkommentiert und konterten mit einer Gegenargumentation „Baclofen and alcohol in France“.

In The Lancet Psychiatry, Roberta Agabio and colleagues published a consensus statement supporting the off-label use of baclofen in the treatment of alcohol dependence as a second-line or first-line pharmacotherapy in patients with contraindication to approved medications, without any mention of the dose. We think that to promote the off-label use of a drug is not appropriate, especially when evidence points to a negative benefit:harm ratio. There is no clear consensus about the use of baclofen, as illustrated by the French situation.
